MEDICAL DIRECTOR Our Physician manages the overall customized comfort care plan for your loved one and visits your home accordingly.
YOUR PHYSICIAN Your Physician is available for consultation with the hospice team.
NURSES Registered Nurses specializing in Hospice Care visit regularly and are available for consultation 24 hours a day.
HOSPICE AIDES Provide assistance with personal care needs such as bathing, dressing, grooming & skin care.
SOCIAL WORKERS Assist with long-term planning and coordination of community services. Additionally they provide assistance with issues such as depression, bereavement and grief support.
CHAPLAINS We serve all faiths. We offer spiritual counseling, prayer and other traditions specific to the patient’s faith. Additionally our Chaplains assist with Funerals, Memorial Services and Celebration of Life gatherings as requested.
VOLUNTEERS Provide respite relief for Caregivers and emotional support for the patient and family. Additionally they can provide a multitude of other support services such as light housekeeping and yard work, meal assistance, errands, pet care and companionship visits.
